Residency and Fellowship Programs
Training Verification
The UC Davis Department of Pathology and Laboratory Medicine is pleased to assist credentialing organizations, licensing boards, hospitals and employers with training verification requests.
To submit a verification request, please provide:
- A completed verification form from the requesting organization
- A signed Authorization and Release of Information form
- Contact information for the requesting party
Requests may be submitted electronically to the appropriate program:
Residency Training Verification

Please allow 30 days for processing, which is required by ACGME.
A $100 verification fee is required for each training verification request. Upon receipt and review of the request, an invoice and payment instructions will be provided.
Integrated AP/CP Residency Program
Core rotations at UC Davis Medical Center and the VA Northern California Health System at Mather Field. Scheduled rotations are integrated throughout the year to include 25 AP months, 20 CP and 3 Electives.
